Your Visit

Doing anything for the first time can be a bit overwhelming. That’s why the team at Custom Physical Therapy and Fitness wants to help you understand what to expect from your time with us.

On your first day, you will complete and turn in the necessary medical forms and insurance information. You can do this upon arrival or in advance with the links below. Regardless of your preferred form submission method, be sure to bring a copy of your insurance card and photo ID.

Once your paperwork is complete and your account is created in our system, you will speak with one of our physical therapists. Because we have a “patients first” philosophy, we take ample time to get to know you. During the conversation, we will review your medical history and work toward a complete understanding of your health status. This means that your therapist will listen to your concerns, ask clarifying questions and observe your movement to see how pain or injury impacts your mobility.

Finally, your physical therapist will review your initial treatment plan with you, including in-office and home care instructions. Your home care plan may include exercises, which will be demonstrated during your visit, as well as the use of ice or heat to ease pain, swelling or stiffness. Of course, we will allow time to answer all questions you may have. Should questions arise after your appointment, simply call or text our office at 704.560.6135.

During visits to our office, we ask that you bring your preferred payment method to cover co-pays required by your insurance company.

Your journey to wellness through physical therapy promises to be a fulfilling experience. If you’re ready to take the first step, complete the form below and a member of our staff will reach out to help you schedule your first visit. Or, if you prefer, you may call or text our office at 704.560.6135.

If you already have an existing appointment, you will find links to our patient forms that you may complete and email back to our office or bring along with you to make your first visit a little easier.

FAQs

How long will my appointment last?
Depending on your course of treatment, your appointment could last anywhere between 45-60 minutes. We encourage our patients to allow ample time in their schedule for therapy as this is something you’re doing to help yourself.
How often will I have physical therapy?
Your treatment plan is custom to your unique needs and will vary from everyone else’s. Most patients can expect between one to three office visits per week in the beginning with daily home care instructions. We conduct periodic re-evaluations to track your progress and adjust as needed.
What do I wear to a physical therapy appointment?
We recommend that patients wear comfortable, loose fitting or stretchy clothing with rubber-soled sneakers for the proper support during manual therapy and exercises.
What can I expect from my treatment sessions?
Your custom care plan will outline what you can expect. As one treatment does not fit all, we may use a combination of therapies during a single session. Some therapies that may be part of your plan include soft tissue and joint mobilization, stretching and exercises. What is most important is that you follow the unique plan designed for you, including all in-office and home care instructions.
Will I always see the same physical therapist?
Yes, except in special circumstances or scheduling difficulties. At Custom Physical Therapy and Fitness, we understand the importance of the doctor/patient relationship and use every opportunity to witness your progress and report any changes.
Can I move or cancel my appointment?
Yes. Custom Physical Therapy and Fitness asks for notice 24-hours in advance of your moved or cancelled appointment. This provides our staff with enough time to contact another patient who may desperately need the space you’re leaving open.
Do you accept health insurance?
Yes. We work with most major insurance providers. If you have a question about your plan, please contact a member of our team and we will do our best to help you find answers.
Is it safe to visit a physical therapy office?
Our office exceeds the cleaning and sanitizing standards set by the CDC. After each use, our tables are deep cleaned with an EPA-registered disinfectant to kill viral pathogens, including coronavirus. Likewise, our entire facility undergoes a deep clean to ensure patient safety.

Patient Success Tip

Don’t skip your appointment when you’re in pain. Communicate openly and honestly with your therapist about your pain level upon arrival so they may adjust your day’s activities accordingly. But, whatever you do, do not stay home.

Patient Success Tip

Whether you want to walk the dog without pain or join your favorite partner for tennis, choose a goal with the help of your therapist, set a specific deadline and stay focused on where you want to be to make tough therapy sessions pass a little easier.

Patient Success Tip

Don’t overdo it. As you gain strength and endurance, it’s tempting to do more reps or lift heavier weights. Resist. Follow your therapist’s instructions to the letter as overdoing it could hinder your progress.

Katie is the best PT I have ever been to see. She treats the whole body and she knows what she is doing. She has provided me with lots of relief.
